SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding a number of forms and information gathering activities prior to their submission for clearance by the Office of Management and Budget (OMB), as well as for some forms and information gathering activities that have already been submitted to the OMB for approval.

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and information gathering activities for which comments are sought are listed in two notices in the April 2 and April 7, 2015, Federal Registers (80 Fed. Reg. 17817 and 80 Fed. Reg. 18669, respecively). Comments may be mailed or faxed to the addresses and numbers listed in those notices.

Comments are due by May 4, 2015, for the following form, which has already been submitted to the OMB for approval: Waiver of Right to Appear-Disability Hearing (SSA-773-U4).

Comments due by May 7, 2015

Comments are due by May 7, 2015, for the following information gathering activity, which has already been submitted to the OMB for approval: Promoting Readiness of Minors in SSI (PROMISE) Evaluation. (Note: This is a correction notice. The SSA inadvertently published the incorrect burden information for this collection at 80 Fed. Red. 3713 on January 23, 2015. The agency now is correcting this error.)

Comments due by June 1, 2015

Comments are due by June 1, 2015, for the following forms, which have not yet been submitted to the OMB for approval: Application for Special Benefits for World War II Veterans (SSA-2000-F6) and Representative Payee Report-Special Veterans Benefits (SSA-001-F6).

Comments due by June 8, 2015

Comments are due by June 8, 2015, for the following forms and information gathering activities, which have not yet been submitted to the OMB for approval: Statement of Claimant or Other Person (SSA-795); Statement of Care and Responsibility for Beneficiary (SSA-788); Request for Internet Services—Authentication; Automated Telephone Speech Technology—Knowledge-Based Authentication (RISA–KBA); and Social Security Number Verification Services (SSNVS).
