SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding a number of new and revised forms and other information gathering activities currently pending with the Administration prior to submission for clearance by the Office of Management and Budget (OMB). Other forms and information gathering activities, as indicated below, have already been submitted to OMB.

Specifically, the SSA wants feedback on the need for the information solicited; its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and other information gathering activities for which comments are sought are listed in two issues of the Federal Register: May 20, 2009 and May 21, 2009. Comments may be mailed or faxed to the addresses and numbers listed in the notices.

Comments due June 19, 2009

Comments are due by June 19, 2009, for the following forms and information gathering activities that have already been submitted to OMB for clearance: Surveys in Accordance with E.O. 12862 for the Social Security Administration (voluntary customer satisfaction assessments, which include paper, Internet, and telephone surveys; mailed questionnaires, focus groups, and customer comment cards); Youth Transition Process Demonstration Evaluation Data Collection (to help young people with disabilities make the transition from school to work); and, the Mental Health Treatment Study (MHTS) (experiments and demonstration projects designed to determine the relative advantages and disadvantages of interventions that facilitate a beneficiary's return to work). The proposed data collection requests appear in the May 20, 2009, Federal Register (74 Fed. Reg. 23764).

Comments due June 22, 2009

Comments on the following proposed data collection requests, which have also been submitted to OMB for clearance, are due June 22, 2009: Request for Workers' Compensation/Public Disability Benefit Information (SSA-1709); and, Prohibition of Payment of SSI Benefits to Fugitive Felons and Parole/Probation Violators (self-reports of applicant's/recipient's status as a fugitive felon or probation/parole violator). The proposed data collection requests appear in the May 22, 2009, Federal Register (74 Fed. Reg. 23916).

Comments due July 20, 2009

Comments on the following proposed data collection requests, which have yet to be submitted to OMB for clearance, are due July 20, 2009: Questionnaire about Special Veterans Benefits (SSA-2010); Request for Reconsideration (SSA-789-U4); Function Report Adult —Third Party (SSA-3380-BK); Disability Hearing Officer's Decision —Title XVI Disabled Child Continuing Disability Review (SSA-1209-BK); Medical Consultant's Review of Mental Residual Functional Capacity Assessment (SSA-392-SUP); Representative Payment Policies Regulation; Function Report Adult (SSA-3373-BK). These proposed data collection requests appear in the May 20, 2009, Federal Register. Also due July 20, 2009, are comments on the following data collection request appearing in the May 21, 2009, Federal Register: Disability Report —Adult (SSA-3368). Note that two items also appearing in the May 21, 2009, Federal Register: Function Report Adult-Third Party (SSA-3380-BK); Function Report Adult (SSA-3373-BK), were duplicated, in error from the May 20, 2009, Federal Register.
