SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding a number of new forms and other information gathering activities prior to submission for clearance by the Office of Management and Budget (OMB) as well as for some activities for which clearance has already been sought.

Specifically, the SSA wants feedback on the need for the information solicited; its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms and other information gathering activities for which comments are sought are listed in the June 9, 2009, Federal Register (74 Fed. Reg. 27379). Comments may be mailed or faxed to the addresses and numbers listed in the notice.

Comments due July 9, 2009

Comments are due by July 9, 2009, for the following proposed data collection requests, which have already been submitted to the OMB for approval: Statement of Employer —to substantiate allegations of wages paid to workers (SSA-7011-F4); Statement of Claimant or Other Person —to obtain information from claimants or other persons having knowledge of facts in connection with claims for Supplemental Security Income (SSI) or Social Security benefits when there is no standard form to collect the needed information (SSA-795); Statement of Self-Employment Income (SSA-766); Certification by Religious Group —for exemption from SECA tax (SSA-1458); Instructions for Completion of Federal Assistance Application —for selection of grant proposals for funding based on their technical merits (SSA-BK-96 ); Representative Payee Report —Special Veterans Benefits (SSA-2001-F6); Request for Deceased Individual's Social Security Record (SSA-711); Request for Business Entity Taxpayer Information —to facilitate direct payment of SSA-authorized fees for representing claimants before the SSA (SSA-1694); Electronic Records Express (online registration process that will allow medical providers to electronically submit disability claimant data).

Comments due August 10, 2009

Comments on the following proposed data collection requests, which have not yet been submitted to the OMB for clearance, are due August 10, 2009: Psychiatric Review Technique (SSA-2506-BK) and Certificate of Election for Reduced Spouse's Benefits (SSA-25).

Previous notice

In addition to the above, the SSA corrected a notice of data collection that had inadvertently appeared in two issues of the Federal Register. The data collection was for the Function Report Adult —for both the report provided by the claimant and by a third party. These notices were published in both the May 20 and May 21, 2009, issues of the Federal Register. The report in the May 20, 2009, was in error. Comment requests on this data collection are due by July 20, 2009.
