SSA seeks comments on proposed information gathering activities

The Social Security Administration is soliciting comments from the public regarding a number of forms prior to their submission for clearance by the Office of Management and Budget (OMB).

Specifically, the SSA wants feedback on the need for the information solicited and its practical utility; the accuracy of the agency's burden estimate; ways to enhance the quality, utility, and clarity of the manner in which the information is collected; and ways to minimize the burden on respondents, including the use of automated collection techniques or other forms of information technology.

The forms for which comments are sought are listed in a notice in the September 25, 2014, Federal Register (79 Fed. Reg. 57650). Comments may be mailed or faxed to the addresses and numbers listed in that notice.

Comments due by November 24, 2014

Comments are due by November 24, 2014, for the following forms, which have not yet been submitted to the OMB for approval: Letter to Employer Requesting Information About Wages Earned By Beneficiary (SSA-L725); Letter to Employer Requesting Wage Information (SSA-L4201); Statement of Living Arrangements, In-Kind Support, and Maintenance (SSA-8006-F4); Claimant's Recent Medical Treatment (HA-4631); Supplemental Security Income (SSI) Claim Information Notice (SSA-L8050-U3); You Can Make Your Payment by Credit Card (SSA-4588 (paper), SSA-4589 (electronic)); Application for Extra Help with Medicare Prescription Drug Plan Costs (SSA-1020 (paper application form), i1020 (online application)); and Certification of Low Birth Weight for SSI Eligibility of Funds You Provided to Another and Statement of Funds You Received (SSA-3380).
